Ontario Disability Support Program Dependent Adults age 18 and over (other than applicant or spouse) and Low Income and Seniors Dental Program

ODSP dependent adults are to phone Special Supports at 905-546-2590
Low Income/ Senior must complete an application to determine eligibility
Participant can choose the dentist of their choice
Participant to provide their dentist with the Special Supports Dental - Approval letter
City of Hamilton sets the Schedule of Dental Services and Fees for this program
